Medical Requirements
All medical information was provided when you registered. The only requirement left is a complete copy of your child's shot record. No need to make an appointment...just call your child's doctor's office and ask for a copy of his/her shot record. This is due to us two weeks prior to your arrival at camp. It may be faxed to 1(800)591-9766 or mailed to Coach Woottens Basketball Camp PO Box 282 Frostburg, MD 21532.
